To register a group for the conference, please email an Excel spreadsheet with the list of attendees to Elizabeth Mueller.

Your spreadsheet should include the following information for each attendee:

  • Organization’s Name
  • First Name
  • Last Name
  • MD or DO Designation
  • Email
  • Phone Number

Additionally, please include the group coordinator’s name and contact information for any follow-up communications.
